Confidentiality Audit Procedure

For each system which contains confidential personal information, the IG Lead will:

• Review logs of failed logins to the system
• Attempt to identify any repeated attempt to access the system

The IG Lead will report annually on

• The result of any known successful access to confidential information by unauthorised persons
• Evidence of shared login sessions/passwords
• Disciplinary actions taken

Reporting confidentiality breaches

Anyone who thinks that there may have been a breach of confidentiality should email hello@openhealthcare.org.uk with a full description.

Breaches of confidentiality requirements

Breaches of Confidentiality requirements will be taken seriously and will result in disciplinary action, including termination of contract for serious breaches.
